The following is a list the of all providers who have registered with the Department, and been certified, to administer vaccine.

Please be advised: this is a comprehensive list of all certified providers, some of whom may have registered with the Department and been approved as providers but never requested a vaccine allotment, and other providers who may have requested a particular vaccine allotment and received some portion of the vaccine they requested.
